Nerlyn Jestine MSN, ARNP FNP-BC

Founder & Chief Executive Officer

Nerlyn Jestine has worked in health care for over a decade. She has provided care to patients that spans from pregnant mothers and newborns all the way to geriatrics. Her experience brings a unique aspect to health care, as she also worked as a Clinical Nurse Case Manager for a Medicare HMO.. During her time working as a Nurse Case Manager,, it became very apparent that there is a gap in our health care delivery system. It was during this time that her vision to provide a way to fill in this gap birthed Customized Care Management Services. She implemented coordination of care modalities by increasing communicating with providers to help decrease readmissions. She also worked closely with skilled nursing facilities to educate them on providing best practice cost containment measures. Her unique skill set and understanding of the importance of clinical oversight, proper education for the patient/family and also the foundation of care coordination gave her a very dynamic perspective. As a graduate of Barry University BSN program she learned the importance of understanding unique communities and wrote a community initiative on a Haitian geriatric population in Miami. At South University where she obtained her Masters in Nursing she worked on an extensive in depth project on pediatric asthma awareness and clinical interventions to reduce readmissions. She is currently an active member with the American Nurses Association and also the National Transitions of Care Coalition. She is board certified through the American Nurse Credentialing Center as a Family Nurse Practitioner.

Dr. Daniel Fortier

Adult/Geriatric Medical Director

Dr. Daniel Fortier is a board certified internist in West Palm Beach, Florida. He
graduated from the SUNY Downstate Medical Center College of Medicine in 1981 and completed his residency at Boston University Medical Center. He currently works in West Palm Beach, FL and specializes in Internal Medicine and Geriatrics at Flagler Medical Center. He is certified through the American Board of Internal Medicine for internal, geriatric and hospice/palliative care. With over 30 years’ experience in medicine, he understands the importance of listening to patients, proper transitional care and collaboration with providers. He currently is affiliated with Good Samaritan Medical Center and is on the Medical Executive Committee. He was awarded the Castle Connolly Top Doctors of Florida in 2009.
